“I have held liquor licenses with the City of Phoenix (#94008384, Tarbell's Restaurant, and #07070163/10076433S, Restaurant Collection) for 23 and 3 years respectively, without indicent. In August 2017 I updated my Title 4 Management Liquor Taw Training Certification.”
