Description
Request to authorize the City Manager, or his designee, to enter into a contract with Banner Occupational Health-Arizona, LLC to provide occupational medicine health staffing and electronic health records management for the Fire Department. Further request to authorize the City Treasurer to accept, and the City Controller to disburse, all funds related to this item. The total value of the contract will not exceed $23,603,544.

Summary
This contract will provide occupational medicine health staffing and electronic health records management for the Fire Department's Health and Wellness Center (HWC), including annual National Fire Protection Association (NFPA) 1580 physicals, recruit physicals, health assessments, infectious control occurrences, injury prevention programs and rehabilitation needs to include physical therapy assessments, evaluations and treatment. This contract will also support local and national research related to health and safety concerns of professional Firefighters and work together with both local and national entities to continue advancements in Firefighter health and fitness. Additionally, this contract supports the Phoenix Police Department with various specialty physicals, Federal Emergency Management Agency (FEMA) operations clearance and outside fire agencies in completion of their required NFPA 1580 annual physicals. The Fire Department will be reimbursed by participating agencies for all associated costs provided to the participating agency.
This item has been reviewed and approved by the Information Technology Services Department.
Procurement Information
A Request for Proposal was processed in accordance with City of Phoenix Administrative Regulation 3.10.
Two vendors submitted proposals deemed responsive and responsible. An evaluation committee consisting of City staff evaluated those offers based on the following criteria with a maximum possible point total of 1,000 points:

After reaching consensus, the evaluation committee recommends award to the following vendor:
Banner Occupational Health-Arizona, LLC, 927.50 points
Contract Term
The contract will begin on or about October 1, 2026, and the initial term will be three years with two one-year options to extend.
Financial Impact
The aggregate contract value will not exceed $23,603,544.
Funds are available in the Fire Department's operating budget.
